Owner Surrender Agreement

The Humane Society of Blue Ridge will accept the surrender of owned cats and dogs by appointment only.  Filling out the form as honestly and completely as you can will help us to rehome your pet.

You will need to provide us with any medical records and medications your animal needs.  Any belongings your pet has that would make them feel more at home would be appreciated.  Also, we need to know what your animal is being fed and how often.

For everyone’s safety all dogs must be leashed and all cats in a carrier.

I understand that HSBR will check the animal’s health and temperament test.  If the animal is deemed aggressive, they will not be accepted.  We make no promises regarding the future placement of your pet.
